    Abstract: Homology Theory on Algebraic Varieties, Volume 6 deals with the principles of homology theory in algebraic geometry and includes the main theorems first formulated by Lefschetz, one of which is interpreted in terms of relative homology and another concerns the Poincaré formula. The actual details of the proofs of these theorems are introduced by geometrical descriptions, sometimes aided with diagrams. This book is comprised of eight chapters and begins with a discussion on linear sections of an algebraic variety, with emphasis on the fibring of a variety defined over the complex numbers. The next two chapters focus on singular sections and hyperplane sections, focusing on the choice of a pencil in the latter case. The reader is then introduced to Lefschetz's first and second theorems, together with their corresponding proofs. The Poincaré formula and its proof are also presented, with particular reference to clockwise and anti-clockwise isomorphisms. The final chapter is devoted to invariant cycles and relative cycles.     Notes: Zusammenfassung Der Beschreibung des Krankheitsbildes der Elephantiasis und differential-diagnostischen Bemerkungen folgt eine Übersicht über die bisher zur Behandlung empfohlenen Operationen, die sich in 3 Gruppen gliedern lassen. 1. Die Eingriffe zur Beeinflussung der Lymphproduktion haben nur mehr historisches Interesse. 2. Den zahlreichen Drainagemethoden ist im allgemeinen nur temporärer Erfolg beschieden. Gleichgültig, ob die Lymphableitung in die Muskulatur und das Knochenmark des Unterschenkels oder in die Subcutis benachbarter Körperabschnitte entlang eingelegter Seidenfäden, Polyäthylenkanülen oder gestielter Hautlappen erfolgen soll, sind nur die vorwiegend ödematösen Formen dieser Behandlung zugänglich. 3. Die Resektionsverfahren sind bei den sklerotischen Formen der Elephantiasis die einzig erfolgreichen. Neben den älteren Methoden, die mehrere Operationen erforderten, werden die vor allem von seiten der plastischen Chirurgie angegebenen radikalen Resektionsverfahren beschrieben. Da bei Verwendung von Spalthaut Insuffizienzerscheinungen der neuen Haut auftreten können, wurde von uns die radikale Exstirpation mit Vollhauttransplatation kombiniert. Das operative Vorgehen wird an Hand eines Falles beschrieben. Bei einer überaus selten zu sehenden Form der Elephantiasis des Penis wurde nach Exstirpation der monströsen Wucherungen der Penisschaft mit einem Spalthautlappen gedeckt. Auch hier was das funktionelle und kosmetische Resultat sehr befriedigend und 2 Jahre nach der Operation kein Anzeichen für ein Rezidiv vorhanden.

Die Organveränderungen zeigen eine deutliche Abhängigkeit von der Dauer der Überlebenszeit, der Dauer der zur Wiederbelebung notwendigen Herzmassage und der Zahl und Stärke der Defibrillationsstromstöße. Im Vordergrund der Frühveränderungen stehen ausgedehnte Herzwandblutungen und schwere Stauungen aller Organe. Das Myokard zeigt bei längerer Überlebenszeit an einzelnen Stellen Verkalkungen und Schwielen. Die Spätveränderungen an den anderen Organen sind uncharakteristisch. Die zum Tode führenden Komplikationen lassen sich in 3 Gruppen einteilen: 1. Akuter Herztod (irreversibles Kammerflimmern in der Wiederbelebungsphase). 2. Gerinnungsstörungen (schwere intrathorakale Blutungen auf der einen und Embolien im großen und kleinen Kreislauf auf der anderen Seite). 3. Spätkomplikationen (in der Hauptsache Pleuraempyem, Pneumonie und versuchsunabhängige Todesursachen). Die möglichen Ursachen der beschriebenen Veränderungen werden diskutiert.

Die Tumoren machen erst subjektive Erscheinungen, wenn sie bereits in Faustgröße palpabel sind. Wichtigstes Diagnostikum ist der Röntgenkontrasteinlauf oder die Magendarmpassage, die eine genaue Lokalisation ermöglichen. Die Pyelographie ist zur Abgrenzung gegenüber retroperitonealen Tumoren wichtig. Es wird auf die große Neigung zu Rezidiven und Generalisation auf das übrige abdominale Fettgewebe hingewiesen, so daß im Gegensatz zu der histologischen Gutartigkeit diese Tumoren klinisch als prognostisch zweifelhaft anzusprechen sind.

    Notes: Zusammenfassung Es interessierte die Frage, ob durch ACTH oder Cortisone eine Änderung der Wundheilung am Magen bewirkt werden kann. Die Klärung der Frage erschien wichtig, da eine verzögerte Wundheilung im Magen-Darmtrakt Ursacheeiner Nahtinsuffizienz sein kann. Da außerdem Arbeiten vorliegen (Eitel undRiecker, Fontaine. u. Mitarb.), in denen auf Grund tierexperimenteller, Untersuchungen bestätigt wird, daß durch Gaben von thyreotropem Hormon als auch durch Thyroxin selbst die Wundheilung begünstigt und z. T. wesentlich beschleunigt wird, sollte auch die Wirkung des bisher weniger bekannten 3,5,3′l Trijodthyronin geprüft werden. Mit dem Skalpell wurde am Magen von Ratten eine bis auf die Mucosa gehende Wunde gesetzt und mit Hilfe histologischer Untersuchungen festgestellt, daß durch Cortisone die Wundheilung am Magen verzögert wird. Dies ist zusätzlich ein Grund dafür, daß ihre Anwendung nur bei strengster Indikation, nach sorgfältigem Abwägen des Für und Wider, erfolgen sollte. Um eine Verzögerung der Wundheilung zu vermeiden, darf die Anwendung von Cortisonen nur eine kurzfristige sein. Ist aber die Gabe von cortisonen über längere Zeit unumgänglich, so sollte zur Vermeidung einer verzögerten Wundheilung zusätzlich Cortexon oder eventuell Trijodthyronin gegeben werden. Bei Ratten, die Trijodthyronin erhielten, war der Wundverschluß durch Serosadeckzellen beschleunigt und die Wundheilung frühzeitig abgeschlossen.

    Notes: Zusammenfassung Das seltene Bild der kongenitalen cystischen Erweiterung des Ductus choledochus (Choledochuscyste) wird beschrieben. Der eigene Fall einer Choledochuscyste, deren Diagnose durch das Auffinden weißer Galle in der Cyste erschwert wurde, wird den bisher etwa 300 veröffentlichten Fällen hinzugefügt. Die Cyste wird unter anschließender Hepatico-Duodenostomie exstirpiert. Eine postoperative Leberinsuffizienz kann unter Anwendung der sog großen Leberkomatherapie abgefangen werden. Dieses Vorgehen wird bei allen Operationen einer Choledochuscyste dringend empfohlen. Die verschiedenen Möglichkeiten der operativen Behandlung der Choledochuscyste werden aufgezeigt.

    Notes: Zusammenfassung Die vorstehende Arbeit beschäftigt sich mit den Veränderungen des AS-Stoffwechsels im posttraumatischen Verlauf. Es kam uns vor allem darauf an, zu zeigen, daß humorale Vorgänge nach Operationen nachweisbar sind, die eine Brücke schlagen zu dem Geschehen im Schock und Kollaps. Wir haben an anderer Stelle über Vermehrungen der Amine und Peptide, vor allem in der Leber, beim Verbrennungs- und Entblutungskollaps berichtet. Ähnliche Veränderungen, wenn auch in weit geringerem Maße, zeichnen sich auch im postoperativen Verlauf ab. Die alte Theorie von der Bildung spezifischer Toxine vom Charakter der Eiweißspaltprodukte ist nach unseren Untersuchungen mit Sicherheit für die niedermolekularen Eiweißbausteine abzulehnen. Dagegen sind physiologischerweise vorhandene Amine und Peptide unter entsprechenden Stoffwechselstörungen, wie vor allem der Hypoxämie, je nach dem Grad des O2-Mangels mehr oder weniger vermehrt. Auch die AS sind selbstverständlich in dieses Geschehen einbezogen. Wie häufig überschneiden und verstärken sich hier Ursache und Wirkung einer Stoffwechselentgleisung. Die Beschäftigung mit den humoralen Vorgängen nach Traumen gibt uns neue Einblicke in die komplizierten Abwehrmechanismen und wird uns hoffentlich auch therapeutisch neue Wege und Möglichleiten erschließen.

    Notes: Zusammenfassung Mit Clearance- und Ausscheidungs-Methoden wurde bei 25 Patienten nach einseitiger lumbaler Sympathektomie in Höhe von L II und L III die Änderung der intrarenalen Hämodynamik untersucht. Diesen Ganglien muß ein nierenwirksames Verhalten zuerkannt werden. Im Gesamtergebnis waren zwar keine signifikanten Abweichungen der Clearance-Größen nachweisbar, es stellten sich aber zwei hinsichtlich der postoperativen Durchblutung unterschiedliche Gruppen heraus, deren gegensinnigen Clearance-Verschiebungen gesichert erschienen. Bei 16 Patienten nahmen der effektive Plasmastrom zu und das Glomerulum-Filtrat ab, die restlichen 9 Patienten wiesen umgekehrte Verhältnisse auf. Die Vermehrung der Nierenblutdurchströmung wird auf eine Erweiterung eines reaktionsfähigen Gefäßsystems bei Herabsetzung des renalen Widerstandes zurückgeführt. Die Ursachen für eine Einschränkung der Nierendurchblutung sind wahrscheinlich in der morphologischen Beschaffenheit der Gefäße auf dem Boden der zugrunde liegenden degenerativen Erkrankungen zu suchen. Für die Veränderungen der intrarenalen Hämodynamik ist bei Systemkrankheiten der Gefäße der Zeitpunkt der lumbalen Sympathektomie von ausschlaggebender Bedeutung.

Das von uns benutzte Verfahren nachSander erlaubt durch Bildung eines dimensionslosen Aciditätsquotienten keine quantitative Beurteilung. Wir beziehen, wie auch andere Chirurgen, den Standpunkt, unter Wahrung der Radikalität und im Hinblick auf postoperative Störungen nur die fakultative Gastrektomie durchzuführen. Bei Infiltration des Carcinoms in benachbarte Organe kann die Operation als Enbloque-Resektion ausgeweitet werden. Eine laufende ärztliche überwachung der magenlosen Patienten ist unbedingt erforderlich.

    Notes: Zusammenfassung Um zuverlässige Anhaltspunkte für die prognostische Beurteilung operierter Kranker zu gewinnen, wurden 100 Fälle von Bronchialcarcinom, bei denen der Eingriff länger als 2 Jahre zurücklag, sowohl morphologisch an Hand der Operationspräparate, als auch klinisch eingehend untersucht. Durch die Einteilung der Tumoren nach ihrer verschiedenen Größe unter gleichzeitiger Berücksichtigung der Symptomendauer konnte dem für die Beurteilung von Malignitätsunterschieden wichtigen Zeitfaktor Rechnung getragen werden. Im Interesse einer genaueren Abgrenzung der verschiedenen histologischen Typen des Bronchialcarcinoms wurden umfangreiche Kerngrößenbestimmungen vorgenommen, mit deren Hilfe es möglich ist, insbesondere den Begriff des kleinzelligen Carcinoms schärfer zu umreißen. Die Gültigkeit der allgemeinen Faustregel, wonach Plattenepithelcarcinome langsamer wachsen und infolgedessen der chirurgischen Behandlung eher zugänglich sind als die undifferenzierten Formen, konnte statistisch bestätigt werden. Praktische Bedeutung gewinnt diese Regel jedoch erst durch die genauere histomorphologische und makroanatomische Analyse jedes einzelnen Falles in Verbindung mit der präoperativen Verlaufsform. Die Möglichkeiten, die Malignitätsunterschiede beim Bronchialcarcinom unter Zugrundelegung der morphologischen Gegebenheiten und des klinischen Verlaufes zu objektivieren, werden an Hand von Beispielen aufgezeigt.

    Notes: Zusammenfassung Anhand von insgesamt 6 Fällen mit einseitigen Uretersteinen, die nach endovesicalen Ureteruntersuchungen an einer einseitigen, ascendierenden, mit Anurie einhergehenden Pyelonephritis erkrankten, wird die Pathogenese und Therapie der Erkrankung besprochen. Durch endovesicale Untersuchungen können hochvirulente Hospitalkeime der Streptokokken-oder Staphylokokkenreihe durch vermutlich aerogen (Atemluft) infiziertes Instrumentarium des untersuchenden Arztes in das gestaute obere harnsystem eingeschleppt werden, die nach wenigen Stunden zu einer foudroyanten Pyelonephritis mit schwersten nephrotischen Veränderungen auch am kontralateralen Organ führen, denen der Kranke ohne Nephrektomie der infizierten Niere stets im anurischen Stadium erliegt. Alle anderen chirurgischen und konservativen Behandlungs-maßnahmen haben den sonst tödlichen Verlauf nicht aufhalten können. Die pathognomonischen Zeichen der Erkrankung sind die harnstauung, die vorausgegangene endovesicale Untersuchung, die septischen Temperaturen, die Leukocytose, die Oligurie oder Anurie, der Nachweis von Streptokokken oder Staphylokokken im Urin und eventuell die Druck-schmerzhaftigkeit der infizierten Niere. Im anurischen Frühstadium (Urin-Tagesmenge etwa 200 cm3) soll möglichst sofort die infizierte Niere freigelegt und bei Bestätigung der Diagnose entfernt werden. Zu diesem Zeitpunkt ist eine Regeneration der vermutlich toxisch-nephrotisch geschädigten Kontralateralniere noch möglich.

    Notes: Zusammenfassung Nach einer kurzen Darstellung der Bauweise des Intestinalsenders für Druck-, Lage- und pH-Wert-Messung werden die, verschiedenen im Magen ablaufenden Druckverhältnisse dargelegt. Mit der gleichzeitig registrierten Lageänderung des Senders kann auf die Bewegungsvorgänge der Ingesta geschlossen werden. Die aus dem Fornix signalisierten Druck- und Lageänderungen entstehen fortgeleitet durch die Zwerchfellbewegung bei der Atmung. Die magneigene Motorik im Korpus läuft in einem 20 sec-Rhythmus ab und führt zu Drucken von 10–20 cm Wassersäule. Im Canalis egestorius beobachtet man häufig eine kurze Folge von Austreibungswellen in einem 2 min-Rhythmus und mit Drucken bis zu 200 cm H2O. Bei der pH-Wert-Messung im Magen werden Valenzen von pH 1,3 bis pH 8,0 registriert. Eine durch Histamineinwirkung ausgelöste vermehrte Säureproduktion findet ihren Ausdruck in den vom Intestinalsender signalisierten Meßwerten, ebenso wie der Übergang in alkalisches Milieu beim Eintreten des Senders in das Duodenum. Neben den mehrperiodischen Druck- und Lageänderungen im Dünndarm mit nur geringen Drucken, wird, zuweilen auch eine zusammenhängende Wellenfolge höherer Drucke im oberen Dünndarm festgestellt, die sich in ihrer Charakteristik von den üblichen Dünndarmschwankungen grundsätzlich unterscheidet. Am ileocöcalen Übergang ist ein Wechsel von mehrperiodischen Dünndarmschwankungen zu einperiodischen Dickdarmschwankungen zu erkennen, ebenso die hier stattfindende Drehung des Senders um seine Achse beim Eintritt in das Coecum. Die Dickdarmschwankungen sind sehr unterschiedlich hinsichtlich Frequenz und Amplitude, einförmig, jedoch in bezug auf ihre Charakteristik. Beim Aufenthalt des Senders in der Flexura hepatica machen sich die von der Leber fortgeleiteten Zwerchfellbewegungen durch atemsynchrone Druck- und Lageschwankungen, bemerkbar. Mit zunehmender Eindickung des Darminhaltes im Dickdarm werden die Lageschwankungen durch Einbettung des Senders in die Kotsäule immer geringer und sind im Sigma nicht mehr feststellbar.
