    Notes: Abstract One of the round tables of the group Futura 2000 at the GECO meeting on 18 January 1999 was devoted to friction couples in total hip prostheses. The discussions were facilitated by the participation of specialists eminent in this field: Professors Sedel and Saillant and Mm Rieker (Société Sulzer), Rack (Société CeramTec), Cales (Société Norton Desmarquest) and Aaron (FHIndustries), to whom we are grateful. We report here on the essence of these discussions.The longterm periprosthetic bony changes are associated with mechanical factors and with biologic factors dominated by the reactions to débris liberated by the prosthesis. Although metal and cement débris contribute to osteolysis, the main factor in bony degradation is the inflammatory reaction to particles of polyethylene [3,4].

    Description / Table of Contents: Summary ATLAS is a cementless, hemispherical, elastic acetabular cup, designed to be impacted. At the start of our experience, the implant was always fixed by screws. Then a screwless fixation by « press-fit » was used, facilitated by its elasticity. Positioning and primary stability are controlled peroperatively by testing. The first 100 cases with 4.5 to 4 years follow-up were studied. Clinical results depend more on the pre-operative status than on the type of implant. We have a high rate of congenital dislocations. We observed no prosthetic failure in this series. Among the 863 ATLAS implanted from 1987 to 1991, 9 failures were observed: 5 revisions for failed total hip replacement, 2 congenital dislocations, 1 rapid destructrive osteoarthritis, and 1 protrusio acetabulum. In all cases a technical mistake could be traced: secondary stabilisation is not possible if the acetabular cup lies only on bone grafts, that is to say devascularized bone. For that reason, we now use, for revisions, an ATLAS cup of greater dimensions (up to 68 mm) with sound bone bearing. The grafts only fill up the lack of bony substance.
    Notes: Résumé Prothèse cotyloïdienne non cimentée, hémisphérique et élastique à impacter, l'ATLAS a évolué depuis sa conception en 1986. Au début de notre expérience il était fixé systématiquement par vis puis la technique de stabilisation sans vis dite du « press-fit », facilitée par l'élasticité de l'implant, fût mise au point. Le positionnement et la stabilité primaire sont contrôlés par des tests per-opératoires. Les 100 premiers ATLAS à 4 ans de recul ont été étudiés. Les résultats cliniques dépendent plus de l'état antérieur, avec fréquence importante des luxations congénitales, que du type d'implant. Nous n'avons observé aucune défaillance du matériel dans cette série. Sur l'ensemble des 863 ATLAS implantés de 1987 à 1991, 9 échecs ont été observés : 5 reprises de prothèse totale, 2 luxations congénitales, 1 CDR, 1 coxarthrose protrusive. Dans tous les cas il s'agissait d'erreurs techniques : la stabilisation secondaire n'est pas possible si le cotyle prothétique repose uniquement sur des greffes osseuses, c'est-à-dire, de l'os dévascularisé. Pour cette raison nous utilisons maintenant dans les reprises des ATLAS de grande dimension (jusque 68 mm) avec un appui sur l'os sain, les greffes venant seulement combler les pertes de substance.
