    Notes: Abstract. Objective: Oxygen consumption (VO2) obtained from respiratory gases by indirect calorimetry (VO2,IC) with a metabolic monitor integrated in a ventilator were to be compared to VO2 obtained by the Fick principle (VO2,Fick ) in septic patients following an increase in oxygen delivery (DO2) induced by positive inotropic support. Design: Prospective clinical study. Setting: University Hospital, Surgical Intensive Care Unit (ICU). Patients: Thirty patients suffering from sepsis. Interventions: DO2 was increased by dobutamine infusion, starting with an initial dosage of 5 µg·kg·min, increased to a maximum of 10 µg·kg·min. Measurements and main results: Dobutamine infusion induced a dosage-related increase in DO2 (from 577±192 to 752±202 ml·min·m2, p〈0.01), which was associated with a statistically significant increase in VO2,IC (from 173±30 to 188±28 ml·min·m2, p〈0.01) and in VO2,Fick (from 140±25 to 156±24 ml·min·m2, p〈0.01). The comparison between VO2,IC and VO2,Fick revealed differences (bias and precision – 33±32 ml·min·m2). Conclusions: With a metabolic monitor integrated in a ventilator it was possible to carry out continuous monitoring of calorimetric data under clinical conditions. In contrast to previous studies using indirect calorimetry, this study showed a moderate correlation between VO2 and DO2 in septic patients using either method. The clinical relevance of this finding requires further investigation. Different factors (e. g. injectant temperature, pulmonary VO2) produced substantial differences between VO2,IC and VO2,Fick as previously shown.

    Notes: Summary The effects of ionizing irradiation on the cochlear microphonic response of guinea-pigs were studied. The cochlear microphonics (CM) of both ears were recorded in a total of 36 animals. Recording was carried out by the differential electrode technique on the basal turn of the cochlea. One week prior to recording, the left ear of each animal had been exposed to 35–70 Gy radiation in increments of 5 Gy. Doses of 40 Gy and above led to a reduction in CM response. After doses of 60 Gy or more, no CM response could be detected. Damage most probably occurred in the outer and inner hair cells.

    Description / Table of Contents: Zusammenfassung Infektiöse Erreger können direkt oder indirekt über das Immunsystem des Wirtsorganismus das Wachstum von Zellen der Gefäßwand modulieren. Die lokale oder systemische Ausschüttung diverser Zytokine während einer Infektionskrankheit könnte deshalb das Ausmaß von Koronareinengungen und Rezidivstenosen wesentlich beeinflussen. Weitere Untersuchungen auf diesem Gebiet könnten zur Erfassung und Behandlung von Patienten mit hohem Risiko für arteriosklerotische Gefäßerkrankungen und Rezidivstenosen nach perkutanen Koronarinterventionen beitragen.
    Notes: Abstract Infectious agents may directly or indirectly (through the response of the host's immune system) modulate the growth of vascular cells. Local and/or systemic increase of cytokines could influence the extent of (re-)stenosis in the vascular tree. Further studies in this field may identify patients at a high risk for atherogenesis and restenosis. Their results should be helpful in treating restenosis after percutaneous coronary interventions.

    Description / Table of Contents: Zusammenfassung In den letzten Jahren gelang es, eine Reihe wichtiger kardiovaskulärer Risikofaktoren zu indentifizieren. Darüber hinaus konnte in einer Vielzahl von Interventionsstudien nachgewiesen werden, daß die kardiovaskuläre Ereignisrate durch entsprechende Interventionen gesenkt werden kann. Es besteht heutzutage kein Zweifel, daß wir den Verlauf der KHK bei Patienten mit einem hohen Risikoprofil durch eine Modifikation dieser Faktoren günstig beeinflussen können. Trotz diesem Wissen werden viele Patienten mit koronarer Herzerkrankung nicht entsprechend der verfügbaren Therapie-empfehlungen behandelt. Die aktuellen Empfehlungen können über das Internet jederzeit abgerufen werden (http://www.chd-taskforce.com). Wir haben das “Marburger KHK-Präventionsprojekt” mit der Zielvorstellung gestartet, daß wir zumindest die sekundärpräventive Versorgung unserer KHK-Patienten verbessern. Wir verknüpfen die Laborwerte des präventivkardiologischen Routinelabors mit den Daten des für die Angiographiebefundung zur Verfügung stehenden Programmes CARDDAS®. Die Patienten werden ebenso wie die behandelnden Ärzte über die Notwendigkeit informiert, die entsprechenden Risikofaktoren zu behandeln. Dieses Vorgehen stellt zumindest sicher, daßs die bei uns diagnostizierten KHK-Patienten eine optimierte Sekundärprävention erhalten.
    Notes: Abstract In recent years we identified numerous cardiovascular risk factors and had been able to reduce cardiovascular events by a variety of different interventions. There is no question that we can improve the course of coronary artery disease (CAD) in patients with a high-risk profile by modifying these factors. Despite this knowledge, many patients with known CAD or myocardial infarction are not treated for secondary prevention as recommended by well established guidelines (http://www.chd-taskforce.com). In order to improve secondary prevention in our patients we started a project, the so called “Marburg CAD Prevention Project”. By this we combine our computer data base of the CAD preventional routine laboratory with the computer program CARDDAS®. Individual risk factors and the angiographic findings were analyzed. Patients as well as local physicians were informed on the need to treat the important risk factors. This approach ensures that at least all of out patients with documented CAD receive the appropriate preventional recommendations and treatment.

    Description / Table of Contents: Abstract Secondary coronary prevention with lipid lowering drugs has become a major issue in health policy formulation due to the large upfront investment in drug therapy. The recently completed LIPID trial with pravastatin in secondary prevention immediately raise the question whether pravastatin might be cost effective in Germany. We conducted a cost-effectiveness analysis from the perspective of third party payers. The following costs were included in the analysis: daily treatment cost of pravastatin, non-fatal myocardial infarction, coronary bypass operations and stroke. Life years gained were obtained by applying the declining exponential approximation of life expectancy. All calculations were standardized to 1,000 treated patients. The net costs of treating 1,000 patients (i. e. drug costs minus the costs of sequelae and interventions) are DM 8,4 Mio. In addition, a total of 405 life years may be saved through treatment. The corresponding cost-effectiveness of pravastatin treatment is DM 20 674,- (DM 17 314,-, discounted by 3% p. a.). The results suggest that the cost-effectiveness of pravastatin in secondary prevention lies well within the threshold of other commonly accepted medical interventions and may be considered an economically viable approach for secondary coronary prevention.
    Notes: Zusammenfassung Entscheidungsträger im Gesundheitswesen sind zunehmen gezwungen, unnötige Kosten zu identifizieren und mögliche Einsparpotentiale im Gesundheitswesen zu realisieren. Gerade bei Krankheiten mit hoher Prävalenz, wie zum Beispiel Herz-Kreislauf-Erkrankungen, kommt hier der Prävention ein besonderer Stellenwert zu. Die vorliegende ökonomische Analyse dient der Klärung der Frage, ob der sekundärpräventive Einsatz des Cholesterinsyntheseenzym-(CSE-)Hemmers Pravastatin bei Patienten mit manifester koronarer Herzkrankheit und Cholesterinwerten zwischen 155 und 270 mg/dl neben dem klinischen Nutzen aud ökonomisch zu vertreten ist. Als Evaluationsform wurde die inkrementale Kosteneffektivitätsanalyse mit dem Endpunkt Kosten pro gerettetem Lebensjahr gewählt. Bei der vorliegenden Untersuchung handelt es sich um eine retrospektive Analyse auf der Basis der LIPID-Studie. Die Berechnung der Kosten erfolgte aus der Perspektive der Kostenträger in Deutschland. Die Nettokosten der Pravastatintherapie, das heißt die Medikationskosten für Pravastatin abzüglich der Kosten für vermiedene Ereignisse, lagen bei rund 8,4 Millionen DM bezogen auf 1000 Patienten in sechs Jahren. Diesen Aufwendungen standen 405 gewonnene Lebensjahre gegenüber. Insgesamt ließen sich Kosten pro gerettetes Lebensjahr in Höhe von DM 20 674,- (DM 17 314,-, diskontiert mit 3% p. a.) errechnen. Der sekundärpräventive Einsatz von Pravastatin bei Patienten mit koronarer Herzkrankheit in Deutschland ist als kosteneffektiv zu bezeichnen.

    Notes: Methodical design of a plant for the production of CaO for use in breathing equipment. The methodology of design nowadays taught at all technical universities can also be profitably applied to chemical engineering. Methodical examination of the production process for CaO used as CO2 absorber in respiratory equipment leads to a considerable improvement in product quality and to more economical production.
