    Notes: High turnover amongst middle managers in organisations can lead to strategic instability as well as operational issues. Since the advent of the NHS internal market flatter management structures have led to middle managers being given more responsibilities along with larger spans of control. This has exacerbated the consequences of the high turnover found amongst service managers. This study examines the relationship between the length of time service managers wish to stay with their organisation, their participation in decision making in directorates, the feedback they receive and their perception of feeling valued. It outlines the findings from a small study and discusses the implications for health trusts.

    Notes: This analysis of the literature examines the implications of the changes in the National Health Service workforce and the ramifications for multi-professional working practices, patient care and delivery of health services. Changes in medical working practices and education, together with the increasing numbers of doctors, have resulted in proposals for nurses to extend their role and the removal of professional restrictions. Whilst the numbers of nurses in the workforce have marginally increased, the qualified nurse ratio has declined, as has the balance between medical and registered nurses. Few attempts have been made to evaluate these changes in relation to the quality of service and patient outcomes. The changing roles of nurses and their value need to be quantified. Demographic trends, together with international shortages and the lack of educational capacity have also been understated. Adequate and accurate workforce information should be regularly collated and analysed.

    Notes: Synexin was isolated from bovine liver and found to aggregate adrenal chromaffin granules in the same Ca2+-dependent manner as previously described for adrenal synexin. The chromaffin granule aggregating activity of liver synexin was blocked in vitro by the addition of an antibody prepared to the 47,000 molecular weight band extracted from an SDS gel of an adrenal medullary synexin preparation. Chromaffin granules aggregated by synexin fused when exposed to cis-unsaturated fatty acids at concentrations comparable to those released from phospholipids by stimulated secretory cells. The synexin-induced aggregation reaction was blocked by Erythrosin B, a common food coloring, and by the phenothiazine antipsychotics trifluoperazine and promethazine. The aggregation and fusion of chromaffin granules thus appears to be a useful model system for studying synexin from diverse tissues and for testing pharmacologically or texicologically active substances for effects on secretory systems.

    Notes: Abstract 4-Amino-p-chlorobutyric acid lactam crystallizes as racemic crystals in the monoclinic space groupP21/n, witha=5.006(2),b=6.783(2),c=27.652(2) Å, β=92.25(1)°,Z=4,R=0.035 for 1618 unique reflections. As both enantiomers are present in a single crystal it is not possible to effect enantiomeric separation by crystal growth inhibition. This lactam may, however, be co-crystallized with (2R,3R)-(+)-tartaric acid forming a 2:1 complex in which only the (R) enantiomer is present. The complex crystallizes in the monoclinic space groupP21 witha=10.074(2),b=10.132(6),c=12.238(6) Å, β=99.13(2)°,Z=2,R=0.036 for 2291 unique reflections. Complex formation is effected by hydrogen bonding but enantioselectivity does not involve the chiral center of the title compound directly.
